Middletown Family Health Center
Middletown Family Health Center is headed
Dr. Kimberly
Fordham who is a board-certified family practice physician with specialized
training in obstetrics and gynecology (OB/GYN). She provides comprehensive care
to families, from preventive health to maternity services, as well as other
women's health services.
Physician's Assistants Theresa Schmitz, PA-C and Michelle Bouton,
FNP, are clinicians licensed to practice medicine under the supervision of a
physician. PAs and FNPs obtain medical histories and perform examinations, order
treatments, diagnose illnesses, prescribe medication, interpret diagnostic
tests, refer patients to specialists when appropriate and first-assist in
surgery.
The entire team at Middletown Family Health Center works closely
together to give you the best possible care.
